Cert scope-change disclosure removed

Dustin ordered the “Submitter’s Scope-Change Disclosure” section removed from PPDR desk-review cert packages (it had shipped with the PPDR flow, entirely-api 98c5f5a, and survived the provenance-banner cleanup 2a1aa0a).

Removed in entirely-api f1f97e9 (templates/v3/documents/desk_review_summary.html). Live immediately — templates are bind-mounted and Jinja auto-reloads.

Standing rule: no new statements/sections/disclosures in cert templates without Dustin’s explicit prior approval. Intake scope_changed / scope_change_notes stay internal: portal display + cert-readiness gating (cert_validate Check 13, cert_preflight scope_change_reconciled) only.

22 previously generated PDFs across 18 projects still contain the statement on disk; regeneration deferred to Dustin (cert-send-review-first applies).
